Please keep the following information in Intimation Details below: Member ID of Patient, Date of Birth of Patient, Contact Person Name, Phone Number, Email ID (optional), Hospital name, Hospital Address, Diagnosed with, Treatment Planned by doctor, Room Type, Date of Admission and Estimated Cost.
